The operation to transplant the Mangekyō went very smoothly. After the surgery, Naruto obtained a complete pair of Mangekyō Sharingan.
Compared to a single Mangekyō Sharingan, a pair is obviously far more taxing. Without the Kyūbi providing chakra, he wouldn't be able to handle it.
At the moment the operation ended, Naruto also learned how to activate Susanoo.

Naruto smiled without replying and continued channeling more chakra. Flesh, blood, and armor gradually formed around the skeletal Susanoo.
Only after reaching the third stage did Susanoo stabilize, transforming into a tall, burly samurai radiating terrifying power.
At that moment, more than a dozen shadow clones burst out from the sand, rushing forward while forming Rasengan.
More than a dozen Rasenshuriken struck Susanoo one after another. The massive shockwaves drove Susanoo into the sand, kicking up a small sandstorm.
Shisui couldn't understand Naruto's actions. Was he testing Susanoo's strength?

The only drawback is that without the Mangekyō, activating Susanoo consumes far more chakra.
Also, only Yin Release chakra can construct Susanoo. In short, it's an advanced ninjutsu suited for the Uchiha—perfect for boosting the combat power of the Uchiha in Uzumaki Village.
Thinking this, Naruto dispelled Susanoo, walked over to Shisui, and brought up the idea.
Shisui nodded. He already knew that Susanoo could be used even without eyes, but he still voiced his concerns and explained why it hadn't been promoted within the clan.
"Lord Mianma, I understand your idea, but Susanoo consumes an enormous amount of chakra. Even jōnin with three tomoe Sharingan can't sustain it for more than twenty seconds. Promoting it wouldn't be very practical."
"Susanoo's size can be controlled. What if we use it like a lightning armor?"
Naruto shared his thoughts.
Shisui fell into deep thought. He had never considered using Susanoo this way. But improving it would require a huge investment of time, effort, and Mangekyō Sharingan…
At that moment, Naruto patted Shisui on the shoulder.
"I've already set up an operating room nearby. I'll transplant the Mangekyō Sharingan for you later. With your talent, you should be able to improve Susanoo."
Feeling the warmth from Naruto's hand, Shisui quickly replied:
"Lord Mianma, you don't need to return the Mangekyō to me. If my eyes can help you, I'm willing—"
Naruto smiled, clasped his hands, entered Sage Fox Mode, and activated Fox Susanoo.
"I don't need the Mangekyō Sharingan anymore. Power at that level doesn't interest me."
Sensing the vast, ocean-like chakra, Shisui was shaken to his core. Every time he thought he had seen the leader's true strength, Naruto revealed something even greater.
Even the Mangekyō couldn't interest him anymore… was Lord Mianma already on par with those legendary ninja duos?
After displaying his power, Naruto contentedly took Shisui to the operating room. His interest in Susanoo was more about learning—understanding how it controlled chakra and laying a foundation for future battles.
After all, Susanoo was essentially a ninjutsu suited for crushing weaker opponents. It was highly effective against enemies at or below Kage level.
But in battles between super Kage-level fighters, it felt almost useless. Kaguya could even shatter a complete Susanoo with a single punch.
Still, Susanoo in this form granted the ability to fly, which Naruto found quite appealing.
He could already fly using Wind Release, but that relied on upward airflow. Susanoo, however, was more like true levitation.
In the blink of an eye, Naruto and Shisui had stayed in the Loulan Oasis for a month.
After relocating most of Loulan's survivors and treating Sara's condition, it was time to return to Uzumaki Village by ship.
But before leaving Loulan, they encountered an unexpected visitor.
The uninvited guest was Orochimaru.
The moment Shisui saw him, his hand went to the sword at his waist. He stepped in front of Naruto, glaring at the infamous snake of the ninja world with hostility.
"Shisui-kun, I mean no harm. I only came to meet Lord Mianma."
Orochimaru flicked out his forked tongue and spoke in a low, eerie voice.
"And I was the one who provided the information on Danzō's whereabouts, wasn't I? We're not enemies."
Naruto raised a hand, signaling Shisui to lower his weapon, then looked at Orochimaru expressionlessly.
"Orochimaru, your intelligence was accurate. But why is Pain also hunting Danzō? I need an explanation."
Orochimaru showed a surprised expression.
"Pain is after Danzō too? How unlucky for him. But you misunderstand me, Lord Mianma. Pain wants me dead—I have no reason to give him information."
"Good."
Naruto snorted coldly, glanced at the sly snake, and said calmly:
"State your purpose. You wouldn't visit the leader of a small ninja village for no reason."
"I came for the Mangekyō Sharingan on Danzō. I want to trade for it with you."
Orochimaru explained his purpose, glancing at Shisui, whose expression had turned extremely ugly.
Naruto frowned, sat down on a stone bench, and said:
"I've already returned Danzō's eyes to their original owner. If you want the Mangekyō Sharingan, you might as well fight Shisui. His strength isn't much different from Uchiha Itachi."
